Family Indemnity Plan (FIP) Checklist
DOCUMENTS NEEDED TO PROCESS ENROLLMENT
- Member's Birth Certificate or Identification Card
- Birth Certificate or Identification Cards for all other persons listed on the form except for children.
- Marriage Certificate - where applicable
DOCUMENTS NEEDED TO PROCESS A CLAIM
- Proof of Death Form - completed by physician
- Member's Birth Certificate or Identification Card
- Birth Certificate or Identification Card of deceased
PLEASE NOTE THE FOLLOWING
- For in-laws to be added, a spouse must be listed on the policy.
- For an adopted child to be added, legal documentation must be submitted.
- Children are removed from policies of parents when they get to the age of 26 years.
- Disabled children are not removed from policies. However a medical certificate must be submitted.
- A Birth Certificate MUST be submitted if a member wishes to add a parent.
- A Birth Certificate must be submitted for step-children
- A Designation of Beneficiary form must be completed if there is only one Insured Person on the Enrollment Form.
- If an insured dies accidentally a Police report MUST be submitted with the claim.
